A 48-year-old man was wounded in a drive-by shooting Sunday afternoon in the Heart of Chicago neighborhood on the South Side.

Someone driving a dark-colored minivan fired shots at the man at 3:28 p.m. in the 2300 block of South Leavitt, according to Chicago Police.

The man was struck in the upper back and taken in serious condition to Stroger Hospital, police said.
