A man died one day after he was shot Sunday evening in the Belmont Central neighborhood on the Northwest Side.

Jordi Ibarra, 23, was found shot in the head at 5:35 p.m. Sunday in the 6200 block of West Grand, according to Chicago Police and the Cook County medical examiner’s office.

Ibarra was taken to Loyola University Medical Center in Maywood, where he died at 3:25 p.m. Monday, according to the medical examiner’s office. He lived in the Belmont Cragin neighborhood.

A police source said the man is a documented gang member.
