A 26-year-old woman was shot Monday evening in the West Englewood neighborhood on the South Side.

She was shot in the left leg and left hand about 5 p.m. in the 6300 block of South Hamilton, according to Chicago Police.

The woman took herself to Holy Cross Hospital, where her condition was unknown, police said.
