A man who died ten days ago after he was shot in the Albany Park neighborhood on the Northwest Side has been identified.

Juan Carlos Olivarez, 43, was driving a van about 2:30 a.m. on May 20 in the 4100 block of North St. Louis Avenue when someone opened fire from a passing vehicle, according to Chicago Police and the Cook County medical examiner’s office.

Olivarez was shot in the back of the head and taken to Illinois Masonic Medical Center, where he died at 3:45 p.m. the same day, authorities said. He lived in northwest suburban Carpentersville.

The man was among 38 people shot in Chicago last weekend.
