A 64-year-old man was killed in a crash involving an ambulance Sunday afternoon in northwest suburban Streamwood.

Jitendra Shah was driving a 2005 Honda CR-V southbound on Route 59 near Tall Grass Court a few minutes after 4 p.m. when the car crossed into northbound traffic and was struck by an ambulance, according to Streamwood police.

Shah, of the 200 block of West Berkley Lane in Hoffman Estates, was taken to St. Alexius Medical Center, where he w as pronounced dead at 5:30 p.m., police said. His passenger, a 53-year-old woman, remains in serious condition at St. Alexius.

The three people in the ambulance were also taken to St. Alexius with non-life-threatening injuries, police said. It was a private ambulance operated by the Trace Ambulance Company.

Police said weather and road conditions may have been a factor in the crash. The MCAT STAR team is investigating.
