West Side man reported missing after discharge from West Suburban Hospital

A 54-year-old man with dementia was reported missing after he was discharged from West Suburban Hospital in Oak Park.

Luis Lopez was last seen leaving the hospital Dec. 28, according to Chicago police. He lives in the 5200 block of West Race Street in the Austin neighborhood.

He’s known to visit the Little Village neighborhood along 26th Street.

Lopez is 5-foot-8 and 180 pounds, police said.

Police asked anyone with knowledge of his whereabouts to call detectives at 312-746-8255.
