A 17-year-old girl has been reported missing from Avondale on the Northwest Side.

Ana Ortiz was last seen Tuesday at her home in the 4000 block of West Nelson Street, according to Chicago police.

Ortiz is 5-foot-8 and 212 pounds with brown eyes and black hair, police said.
