Chicago police have released a photo of an SUV wanted in connection with a hit-and-run last year in Logan Square on the Northwest Side.

About 2:50 p.m. on Dec. 6, a blue Chevrolet Trailblazer struck a 41-year-old man in the 2000 block of North Kedzie Avenue, seriously injuring him, Chicago police said.

Anyone with information can contact the Major Accident Investigation Unit.
