A 13-year-old girl has been reported missing from Portage Park on the Northwest Side.

Julina Hernandez was last seen about 11 p.m. Monday in her residence in the 5300 block of West Addison Street, according to a missing person alert from Chicago police.

Hernandez is 5-foot-1, 155 pounds, with brown eyes, brown hair and has an olive complexion, police said.
