A man was shot dead Wednesday evening in the East Garfield Park neighborhood on the West Side.

The 45-year-old was found unresponsive with a gunshot wound to the head at 7:21 p.m. and pronounced dead at the scene in the 3000 block of West 5th Avenue, according to Chicago Police.

The Cook County medical examiner’s office could not immediately confirm the fatality Wednesday evening. The circumstances of the shooting were unknown.
