A woman was killed and six people — including five children — were injured in a crash on Interstate 80/94 in the south suburbs Monday evening.

A van carrying two adults and five children collided with a semi truck in the westbound lanes of the interstate near Route 394 in South Holland about 5:20 p.m., according to Illinois State Police.

The van rolled over during the crash and the driver, 70-year-old Mary L. Kulovitz, was taken to Ingalls Memorial Hospital in Harvey, where she was pronounced dead at 6:22 p.m., according to police and the Cook County medical examiner’s office. Kulovitz lived in the 11200 block of Poplar Creek Lane in Orland Park.

Another adult and five children were hospitalized with injuries not believed to be life-threatening, police said.
