A federal report detailed the miscommunication and errors that led to the death of Chicago Fire Department Capt. Herbert Johnson at a 2012 house fire.

The blaze that took Johnson’s’ life saw a number of factors that led to the tragedy according to findings from the Fire Fighter Fatality Investigation and Prevention Program. The report and findings are below:
