Bolingbrook metal stamping facility fined $110K by OSHA

A southwest suburban metal stamping company faces more than $110,000 in fines after being cited for several serious safety violations by the U.S. Department of Labor.

D&D Manufacturing Inc. at 500 Territorial Dr. in Bolingbrook was cited for seven repeat, six serious and four other-than-serious violations after an October inspection, according to a statement from the Occupational Safety and Health Administration. The inspection was prompted by a complaint received by OSHA.

The inspection found that the company had failed to report an amputation injury to OSHA within 24 hours, according to the agency. The company had also failed to provide proper machine guarding, locking and blocking devices to prevent unexpected machine starts and operation; training on machine hazards; and safety procedures and proper storage of compressed gas cylinders. Workers were also exposed to live wires and damaged electrical equipment.

OSHA has proposed penalties of $110,000 and placed D&D in its Severe Violator Enforcement Program, according to the agency.

D&D, which has its corporate headquarters in Bolingbrook, “fabricates metal components for equipment manufacturers” and employs 75 workers in Illinois, according to the statement.

The violations found at the Bolingbrook facility are similar to violations discovered less than three years ago at the company’s facility in El Paso, Texas.
