A 19-year-old man was shot Sunday evening in the Humboldt Park neighborhood on the West Side.

He was shot in the chest about 7:35 p.m. in the 3000 block of West Grand, according to Chicago Police.

The man took himself to Saints Mary and Elizabeth Medical Center in an unknown condition, police said.
